Congratulations on Completing the

2026 Nutrition BINGO Challenge!

Voucher Upload Instructions:

  1. Log into your Sanford Health Plan Account
  2. Click on Portals and Links
  3. Click on the Wellness Portal
  4. Find the Rewards section and click Unlock Program
  5. Scroll down to More Activities and click on Workplace Wellness Initiatives
  6. Click on Worksite Voucher Award #1 to enter in your first workplace voucher
  7. Click Report Completion
  8. Enter in the Date of the Challenge found on the NDPERS Voucher
You can upload a total of 4 - 3000 point vouchers. Work Well Challenges (like the Level Up Challenge) count as 6000 points...so you would upload these events twice.

*Only people who have participated in 60% of this event are able to claim these points.
*Spouses are not eligible for worksite wellness vouchers, UNLESS they also work for the
State of ND.

Note: The $250 Wellness Benefit is considered taxable income and applicable taxes will be automatically be taken out of your paycheck once the reward is claimed.
