We are so excited to cheer you on as you navigate residency, and your programs are incredibly lucky to have you! The Office of Student Affairs and Admissions is happy to have been a just a small piece of your experience and we hope that you stay in touch!

Degree Verifications for MS4s

Congratulations on almost completing your degree! We are so excited for all of you! Many of you may need verification forms completed, transcripts, diploma copies, and a variety of other documentation for your residency programs or next steps. Please submit these requests through the Graduating Medical Student Degree Verification Request. You will need to sign in with your NDUS email account to complete this form. We ask that you try to submit all the items/documents you need complete on one form, but multiple can be completed if necessary. Please name any files that you submit using your last name, state, and the form title (i.e. Jones ND Med Ed Form).

If we need more information from you, we will send you an email and you may need to complete another submission.

*Please note that we cannot send any information to your programs until after graduation and your degree has been finalized. This may take a few weeks while we wait for all final grades to be posted and have your transcripts certified by the Registrar’s Office. Once this happens, we will send the documents to the address provided. Once the forms have been completed by our office, you will receive an email from us letting you know.

**SAA is not able to send official transcripts. UND uses Parchment.com to send these. Once your degree has posted to your diploma, you will receive an email with instructions for ordering them.

If you have already sent an email to SAA staff with a request, please submit your request using the Graduating Medical Student Degree Verification Request so that it goes into the correct folder to be tracked. Please let SAA know if you have any questions or concerns.
